DUTIES:

  • Reporting to the Manager of Nutrition & Dietary Services, this position will be responsible for planning, coordinating and evaluating the clinical aspects of all residents’ nutritional care in accordance with the Ministry of Health Act/regulations, the standards of practice of the College of Dieticians of Ontario /Canada Code of Ethics and Professional Misconduct Regulation, and other applicable legislation.
  • Ensures that all nutrition care standards are met and that the resident receives quality nutritional care. Will manage the nutritional needs of all residents through nutritional risk screening, assessments, referrals, individualizing nutrition care planning, prescribing appropriate nutritional interventions, analyzing, observing resident meals; monitoring weights and develop appropriate action plans continued follow up and evaluation of outcomes as mandated by the Ministry of Health and Long Term Care.
  • Will perform diet counselling consultations as requested by nursing staff, physician, and residents’ family members. Conducts resident dietary review through the RAI-MDS/RAPS process on a quarterly basis at minimum; assessments include review of all lab work, doctor’s notes, medical chart notes, food and fluid intake sheets, hospital discharge notes, weight review, diet texture review, assistive device review, nutritional risk review, and any interventions that have transpired over the previous quarter; ensuring cycle menus for regular and therapeutic diets meet nutritional standards; eating and swallowing assessments for restorative feeding and texture modification; conducts meal rounds to assess residents’ nutritional requirements and status ; evaluates the effectiveness of nutritional care interventions; manages nutrition related performance indicators.
  • Assists the Manager of Nutrition/Food Services in the quality improvement/management/control and audits mandated by the Ministry Inspection IPs. This position will oversee the nutritional component of the facility’s enteral feeding program; will assist in the development of the nutritional care procedures specific to the residents’ nutritional care.
  • Maintains open communication with dietary, nursing and other support staff to ensure all information is conveyed efficiently and in a timely manner; will prepare and distribute diet cards to pertinent staff and prepare facility and resident memos as required; Directs staff on diet and nutritional requirements of residents. Liaises with the Ministry of Health and Long Term Care, Dieticians of Canada, hospitals, other long term care facilities and other accredited professionals to ensure compliance and minimize risk.  Develops, plans and conducts staff training on full review of nutritional care; oversees the implementation and evaluation of procedures and compliance with the directives of nutritional care; compiles and presents statistics.
  • Participates in all care conferences, Professional Advisory committee, Palliative care program, Bowel management, restorative care, Dysphasia management etc. and adjusts care plans as needed.
  • Will perform Occupational Health & Safety duties as outlined in the Corporation’s Health and Safety Program.  Will perform other related duties as required.

QUALIFICATIONS:

  • Must have a University Degree in Food and Nutrition or Ontario Ministry of Education equivalency and a minimum of one (1) year of experience in nutrition and dietetics.
  • Must be a licensed member in good standing as a Registered Dietician as recognized by the College of Dieticians of Ontario;
  • Must be proficient in the use of computers and various software systems;
  • Must be available for variable shifts;
  • Should have knowledge of the Occupational Health and Safety Act, its regulations and knowledge of the hazards associated with the work.
  • Work experience in a health care setting a definite asset;
  • Clinical experience as a Registered Dietitian in a long-term care setting will be considered an asset;
  • Given that this job is a new position a Physical Demands analysis will be completed with the incumbent after approximately six months of occupying the position.
